• Attach self-attested copies of required documents (ID proof, marksheets, certificates, etc.).
• Submit the form in-person at the DLSA Office, Gwalior, or send it via Registered Post to the mentioned address.
• Ensure the form reaches the office before the last date – 19.12.2025 (5 PM).
